This page explains the financial responsibility and payment policies for services provided by Inspired Aesthetic Medicine ("the Practice"). By scheduling or receiving services, patients agree to the policies below.
Identification
Patients must provide picture identification to the receptionist for photocopying at each appointment.
Payment
Inspired Aesthetic Medicine is a self pay practice. Payment in full is due at the time of each service.
All services rendered are charged directly to the patient, who is personally responsible for full payment. If treatment is suspended or terminated, any fees for professional services rendered to the patient or their dependent up to the point of termination are immediately due and payable. Patients are responsible for any outstanding fees for services provided by the Practice.
Payment Plans and Deferrals
Any arrangement involving a payment plan or payment deferral must be made in writing with the office manager or business manager of the Practice. Verbal agreements are not accepted.
Cancellations and No Shows
The Practice reserves the right to charge a $100 fee for a missed appointment, or for an appointment changed or canceled with less than 48 hours notice.
Late Arrivals and Incomplete Forms
The Practice reserves the right to reschedule an appointment if a patient arrives more than 10 minutes late or has not completed their forms prior to the appointment time. Appointments rescheduled due to late arrival or incomplete forms will incur the $100 cancellation fee.
